JOB TITLE: Social Work Case Manager - Brooklyn & Newark
LOCATION: Newark, NJ & Brooklyn, NY
REPORTS TO: Director, Youth & Support Services

Per Scholas is seeking an enthusiastic, mission-driven, expert Social Worker to work with students by providing specialized support to ensure their success in the training program. In this dynamic role, you will provide comprehensive case management, benefits navigation, & targeted support to students participating in our training & bridge programs.  As an integral member of our Youth & Support Services team you will:

  • Address students short- & long-term challenges by providing direct intervention & referrals to social service agencies that are responsive to their unique needs. Follow up with students & service providers to monitor outcomes & ensure appropriate services are provided
  • Research & develop additional resources & partnerships across the city that can provide external referrals to students & alumni
  • Navigate through challenges related to public benefit programs & social services & build students self-advocacy skills
  • Support the day to day recruitment, training, & programming for bridge programs 
  • Meet programmatic goals & compliance as required by Per Scholas partners & funders


Case Management

  • Help students understand, access, & maintain public benefits & other supports while enrolled in Per Scholas & develop comprehensive personal post-graduation plans.
  • Provide individualized & tiered support to students in our courses. Document contact & meetings in real-time, using case notes in
  • Connect students with outside referrals as necessary, follow-up & track.
  • Conduct ongoing assessment of students strengths & challenges throughout all phases of the program
  • Counsel around the removal of barriers that impede engagement & retention in each phase (i.e. housing, family support, healthcare access)  
  • Work closely with graduates on their personal & financial goals for up to 2 years post graduation in conjunction with the Career Advancement team

Program Support

  • Work collaboratively with other team members, including career coaches, technical instructors, & business solutions team to support students path to graduation & post-graduation employment
  • Manage program-related budgets to provide monetary resources to students & alumni as needed
  • Develop & organize a comprehensive on-site resource center with articles, tip sheets, worksheets, & other information to better inform our students
  • Build & support partnerships with community-based organizations & government agencies, to enable credible referrals 
  • Track student progress & complete weekly reports based on student meetings & critical financial indicators, while ensuring the privacy, confidentiality & data security of all students
  • Create, analyze & present regular progress reports, to supervisors, partners, funders
  • Participate in local & national events, forums & conferences related to navigation, case management, & financial education
  • Stay current on developments in the social work & financial education field

Professional Qualifications

  • Master of Social Work degree required
  • LMSW/LSW preferred or planning on earning licensure within six months of hire
  • At least 3 years experience providing effective, efficient case management support in a social services environment
  • Knowledge of trends & issues, laws & regulations related to the development & delivery of case management services preferred
  • Experience in organizational training & development or learning & development preferred
  • Demonstrated experience in facilitation & classroom instruction & understanding principles of classroom management & diversity
  • Experience training adults (18+) a plus
  • Knowledge of workforce development a plus
  • Ability to establish relationships with individuals & organizations of influence within the social services community, including civic groups, charitable agencies, related government entities, fundraising sources, etc.
  • Experience measuring, reporting, & communicating program results
  • Strong computer skills, including Word, Excel, PowerPoint & Google Suite; Salesforce & Change Machine experience a plus
  • Ability to work occasional evenings
