Role Compensation

  • Total compensation $80,500 (includes 15% target bonus, paid quarterly, based upon performance as assessed by management). 
  • Compensation commensurate with credentials & experience. 

About the Team & Opportunity: 

Quartet is looking for a Care Manager - Social Worker in the Philadelphia area who is passionate about holistic care to those with serious mental illness. Our Care Manager - Social Worker is a critical member of our multidisciplinary team, responsible for referral, linkage & related coordination of care for managing behavioral care for adult patients with serious mental illness, both virtually & in person. 

Accountabilities:

  • Develop a strong rapport with a diverse set of patients (socioeconomic status, age, sexual orientation etc.), supporting patient engagement in the program
  • Responsible for inbound & outbound patient contact, including scheduling appointments & managing inbound phone calls directly from patients 
  • Liaisons to facilities (e.g. hospitals, inpatient centers) by developing deep relationships with key personnel (e.g. Discharge planners)
  • Determines plan to address non-clinical care gaps by facilitating connections to community-based services & supports
  • Flags acute events such as patient hospitalizations to the Nurse Care Manager
  • Leads patient management services including medical records requests, prior authorizations, pre-certifications, advanced care planning documentation, etc.
  • Willingness to assist with after hours call coverage on an as-needed basis, in partnership with the rest of the care team in a staggered schedule

Minimum Qualifications

  • Bachelors or Masters Degree in Social Work or LCSW
  • 2+ years of clinical social work experience with behavioral health populations, or 5+ years of clinical social work experience with general populations
  • Experience in supporting longitudinal care plans for complex patients, in partnership with a broader internal Care Team
  • Ability to travel locally within the Philadelphia region at least 60% of the time to see patients in the hospital, home & community
  • Have a valid drivers license, active auto insurance, have access to a car & willingness to drive to the patient location
  • Excellent written & verbal communication with an emphasis on clarity & compassion
  • Strong computer skills, including experience working with Electronic Health Records (EHR)
  • Basic understanding of insurance products, benefits, coverage limitations, laws & regulations as it applies to the health plan

Preferred but not required Qualifications

  • Experience in supporting care management for individuals with complex mental health concerns, including patients with Serious Mental Illness or co-occurring substance use disorders
  • Experience working with academic health systems
  • Knowledge of the local health system landscape & available providers in the greater Philadelphia area

  • Experienced in using Microsoft Word & Excel as well as ability to work with multiple applications
  • Bilingual candidate (Spanish and/or other languages)
  • Passionate about helping build innovative models of care that improve the lives of vulnerable populations
